  1. Describe Mercy's ambulatory palliative care service model that results in better patient symptom management, timely goals of care conversations and avoidance of unnecessary hospitalizations.

  2. Build the business case for outpatient palliative care services and the professional resources needed to provide comprehensive palliative care services that meet patient/family needs.

  3. Discuss unique aspects of a pediatric palliative care program that encompass inpatient, ambulatory, residential and home encounters for children with complex medical issues over an extended period of time.
