Learning Objectives:

  1. Describe some unique challenges faced by persons experiencing homelessness at end-of-life and how health care providers might adapt their practices to address these challenges.

  2. Learn about one innovative model of care that was created as a result of a partnership between hospice and homeless health care providers in a large urban community.

  3. Consider the practical issues that must be addressed in initiating and sustaining a service like this in a local community, who might the stakeholders be, and what resources exist to support an effort such as this.
